Now her work has led to another giant leap forward with a first-in-the-world clinical trial in Spain involving children who are receiving heart transplants.<\/p>\n\n\n<div class=\"wp-block-media-text has-media-on-the-right is-stacked-on-mobile\" style=\"grid-template-columns:auto 25%\"><div class=\"wp-block-media-text__content\">\n<p class=\"wp-block-paragraph\">\u201cWe can make a significant impact in the field of transplantation,\u201d emphasizes Dr. Levings, who is also a professor at the University of British Columbia\u2019s Department of Surgery and School of Biomedical Engineering and the Canada Research Chair in Engineered Immune Tolerance. \u201cWhen I get the opportunity to meet patients and families, I am reinvigorated to work harder and faster because I know they are counting on research to make a difference.\u201d<\/p>\n<\/div><figure class=\"wp-block-media-text__media\"><img loading=\"lazy\" decoding=\"async\" width=\"600\" height=\"900\" src=\"https:\/\/www.bcchr.ca\/levingslab\/files\/2025\/01\/megan-levings-web.jpg\" alt=\"\" class=\"wp-image-2369 size-full\" srcset=\"https:\/\/www.bcchr.ca\/levingslab\/files\/2025\/01\/megan-levings-web.jpg 600w, https:\/\/www.bcchr.ca\/levingslab\/files\/2025\/01\/megan-levings-web-200x300.jpg 200w\" sizes=\"(max-width: 600px) 100vw, 600px\" \/><\/figure><\/div>\n\n\n<p class=\"fndry-paragraph\"><br>Tregs, as they are known, help orchestrate the rest of the immune system by ensuring the body doesn\u2019t attack something it shouldn\u2019t. Some of the first evidence for their existence was reported in 1969 and Tregs have been the subject of much study since then. The pediatric immunology researcher and&nbsp;<a href=\"https:\/\/inmunoregulacion.es\/quienes-somos-2\/\" target=\"_blank\" rel=\"noreferrer noopener\">head of the immunoregulation laboratory<\/a>&nbsp;at the Gregorio Mara\u00f1\u00f3n Health Research Institute in Madrid had also been studying the immune systems of children with heart transplants, trying to find a way to prevent rejection without the need for drugs.<\/p>\n\n\n<div class=\"wp-block-media-text has-media-on-the-right is-stacked-on-mobile\" style=\"grid-template-columns:auto 25%\"><div class=\"wp-block-media-text__content\">\n<p class=\"wp-block-paragraph\">Dr. Rafael Correa-Rocha is one of those collaborators. The pediatric immunology researcher and&nbsp;<a href=\"https:\/\/inmunoregulacion.es\/quienes-somos-2\/\" target=\"_blank\" rel=\"noreferrer noopener\">head of the immunoregulation laboratory<\/a>&nbsp;at the Gregorio Mara\u00f1\u00f3n Health Research Institute in Madrid had also been studying the immune systems of children with heart transplants, trying to find a way to prevent rejection without the need for drugs.<\/p>\n<\/div><figure class=\"wp-block-media-text__media\"><img loading=\"lazy\" decoding=\"async\" width=\"600\" height=\"900\" src=\"https:\/\/www.bcchr.ca\/levingslab\/files\/2023\/05\/rafael-correa-rocha.jpg\" alt=\"\" class=\"wp-image-2454 size-full\" srcset=\"https:\/\/www.bcchr.ca\/levingslab\/files\/2023\/05\/rafael-correa-rocha.jpg 600w, https:\/\/www.bcchr.ca\/levingslab\/files\/2023\/05\/rafael-correa-rocha-200x300.jpg 200w\" sizes=\"(max-width: 600px) 100vw, 600px\" \/><\/figure><\/div>\n\n\n<p class=\"fndry-paragraph\"><br>Currently, transplant recipients must take drugs for the rest of their lives to suppress their immune system so it doesn&#8217;t attack the donated organ. However, the drugs are toxic and come with a long list of potential side effects, including an increased chance of infections, cancer, kidney problems, diabetes and high blood pressure. Even with the medication, rejection remains a significant problem leading to high mortality in children with heart transplants. The median survival is about 15 years.<\/p>\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p class=\"wp-block-paragraph\"><\/p>\n<\/blockquote>\n\n\n<div  class=\"fndry-container fndry-responsive-bg fndry-responsive-border fndry-pt--3 fndry-pb--3 fndry-pr--md-1 fndry-pl--md-1 fndry-pt--sm-0 fndry-pb--sm-0 fndry-mt--sm-3 fndry-mb--sm-3\">\n\t<div class=\"fndry-row\">\n\t<div  class=\"fndry-col fndry-responsive-bg fndry-col--12 fndry-col--sm-12 fndry-col--md-12 fndry-pl--sm-0\">\n\t<div  class=\"fndry-container fndry-responsive-bg fndry-responsive-border fndry-pt--3 fndry-pr--3 fndry-pb--3 fndry-pl--3 fndry-pt--sm-2 fndry-pr--sm-2 fndry-pb--sm-2 fndry-pl--sm-2\" style=\"--fndry-bg:var(--fndry-color-light)\">\n\t<figure  class=\"fndry-quote fndry-quote--left\">\n\t<blockquote class=\"fndry-quote__quote\">\n\t\t<p class=\"fndry-text-intro22\">\u201cIf we can completely remove immunosuppression drugs and use Tregs to achieve immune tolerance, the state where the body tolerates the transplanted heart and doesn\u2019t mount an immune response, then we have achieved the Holy Grail.&#8221;<\/p>\t<\/blockquote>\n\t<figcaption  class=\"fndry-quote__citation\">\n\t\t&#8211; Dr. Rafael Correa-Rocha\t<\/figcaption>\n<\/figure><\/div>\n<\/div>\n<\/div><\/div>\n\n\n<h3 class=\"fndry-heading\">Finding a pediatric solution<\/h3>\n\n<p class=\"fndry-paragraph\">But there was a significant roadblock to developing Treg therapy in children. In research involving adult patients, Tregs were generated from the participants\u2019 own blood and then transfused back into the patient. This method requires 500 millilitres, or two cups of blood, which is too much to take from babies or children. There had to be another source of Tregs that would work for pediatrics. Working together, Dr. Levings and Dr. Lori West at the University of Alberta found it.<\/p>\n\n<p class=\"fndry-paragraph\">\u201cDuring heart surgery on infants, thymuses are routinely removed to allow the surgeon better access to the heart. Turns out those discarded thymuses, which were just ending up in the garbage, are actually an excellent source of high-quality and potent Tregs,\u201d says Dr. Levings. \u201cOne gram of thymus has 500 times more Tregs than one millilitre of blood.\u201d<\/p>\n\n<div  class=\"fndry-container fndry-responsive-bg fndry-responsive-border fndry-pb--3 fndry-pt--3 fndry-pr--md-1 fndry-pl--md-1 fndry-pt--sm-0 fndry-pb--sm-0 fndry-mt--sm-3 fndry-mb--sm-3\">\n\t<div class=\"fndry-row\">\n\t<div  class=\"fndry-col fndry-responsive-bg fndry-col--12 fndry-col--sm-12 fndry-col--md-12 fndry-d--flex fndry-flex--col fndry-align--start fndry-justify--center\">\n\t<figure  class=\"fndry-image\" style=\"--imageWidth:100%;--img-height-all:675px\" aria-labelledby=\"img-2455\">\n\t<img loading=\"lazy\" decoding=\"async\" width=\"900\" height=\"600\" src=\"https:\/\/www.bcchr.ca\/levingslab\/files\/2023\/05\/lori-west-megan-levings.jpg\" class=\"fndry-image__img\" aria-hidden=\"true\" role=\"presentation\" style=\"--borderRadius:0px;--objectFit:cover;--imagePosX:50%;--imagePosY:50%\" srcset=\"https:\/\/www.bcchr.ca\/levingslab\/files\/2023\/05\/lori-west-megan-levings.jpg 900w, https:\/\/www.bcchr.ca\/levingslab\/files\/2023\/05\/lori-west-megan-levings-300x200.jpg 300w, https:\/\/www.bcchr.ca\/levingslab\/files\/2023\/05\/lori-west-megan-levings-768x512.jpg 768w\" sizes=\"(max-width: 900px) 100vw, 900px\" \/><\/figure>\n<div  class=\"fndry-container fndry-responsive-bg fndry-responsive-border fndry-pt--2 fndry-pl--2 fndry-pr--2\" style=\"--fndry-bg:var(--fndry-color-light)\">\n\t<p class=\"fndry-paragraph\">Dr. Levings (on the right) has been collaborating with Dr. Lori West from the University of Alberta on several research projects in the field of immunology specific to transplant recipients.<\/p><\/div>\n<\/div>\n<\/div><\/div>\n\n\n<p class=\"fndry-paragraph\">In 2016, Dr. West and Dr. Levings published their&nbsp;<a href=\"https:\/\/www.amjtransplant.org\/article\/S1600-6135(22)00513-5\/fulltext#%20\" target=\"_blank\" rel=\"noreferrer noopener\">groundbreaking study on thymic Tregs<\/a>. It was perfect timing for where Dr. Correa-Rocha was at with his work. Together, they agreed Spain was the ideal place to move this idea from the lab into humans for the first time. Spain is the world leader in organ transplantation with around 25 to 30 pediatric heart transplants each year.<\/p>\n\n<h3 class=\"fndry-heading\">From the lab into humans for the first time<\/h3>\n\n<p class=\"fndry-paragraph\">Dr. Correa-Rocha then developed a protocol to test the safety of infusing thymic Tregs into children who are undergoing a heart transplant. During surgery, the surgeon removes the thymus, the research team then takes the thymus and&nbsp;<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/35651624\/\" target=\"_blank\" rel=\"noreferrer noopener\">grows a large volume of Tregs<\/a>, which is required for therapeutic use. The thymic Tregs are also high-quality pure cells, compared to Tregs manufactured from other sources. The expansion process takes about a week, then the cells are infused into the patient. The initial induction therapy immediately post-transplant for immune suppression has been removed, but daily medications will follow current protocol.<\/p>\n\n<p class=\"fndry-paragraph\">\u201cThese cells have never been used in a clinical trial before, especially in children,\u201d says Dr. Correa-Rocha. \u201cIt took two years for the Spanish medical authority to approve the study. Then COVID-19 hit.\u201d<\/p>\n\n<p class=\"fndry-paragraph\">While heart transplants didn\u2019t halt completely during the initial wave of the pandemic, the numbers of surgeries decreased considerably. Irene&#8217;s story captured a lot of media attention.<\/p><\/div>\n<\/div>\n<\/div><\/div>\n\n\n<p class=\"fndry-paragraph\">Irene was just six months old when she received a heart transplant. A week later, Treg cells expanded from her own thymus were infused into her. So far, the&nbsp;<a href=\"https:\/\/journals.lww.com\/transplantjournal\/Citation\/2022\/09001\/213_2__Preliminary_Results_of_the_First_Clinical.45.aspx\" target=\"_blank\" rel=\"noreferrer noopener\">now three year old is doing very well<\/a>.<\/p>\n\n<p class=\"fndry-paragraph\">\u201cThe results are amazing. After two years of study followup, Irene has shown no sign of rejection. And she has also maintained full levels of Treg cells following that one infusion,\u201d says Dr. Correa-Rocha.<\/p>\n\n<p class=\"fndry-paragraph\">Compare that to pediatric heart transplant recipients who haven\u2019t undergone the thymic Treg therapy. After two years, their Treg levels are 80 per cent lower than before transplantation. If levels can be maintained through this novel therapy, the theory is recipients wouldn\u2019t need those toxic medications to control rejection. The Tregs would provide a natural way to police the immune system.<\/p>\n\n<h3 class=\"fndry-heading\">Opening the door to a new generation of immunotherapy<\/h3>\n\n<p class=\"fndry-paragraph\">It is still early stages. Four more babies have received the thymic Treg therapy since Irene, and another three children and their families have consented to taking part in the study and are awaiting a heart transplant. If the treatment continues to prove to be safe, the next step would be to enrol these pioneers into a new clinical trial. Children who are able to maintain normal levels of Tregs would slowly reduce their immune suppression drugs. For example, thymic Tregs could also be used in different individuals, children and adults, to control rejection, or treat other types of immune responses. The large volume of Tregs that can be manufactured from a single thymus would allow for hundreds of therapeutic doses.<\/p>\n\n<p class=\"fndry-paragraph\">Dr. Correa-Rocha just received approval for a study in Spain using thymic Tregs in patients hospitalized with COVID-19 who are undergoing a severe immune reaction.<\/p>\n\n<p class=\"fndry-paragraph\">Here in Canada, Dr. Levings is launching a first-in-human clinical trial of thymic Tregs to prevent graft versus host disease, a severe side effect from a bone marrow or stem cell transplant. That\u2019s amazing.
