The consortium leads global maternal health research aimed at developing, testing and introducing new knowledge that will reduce the unacceptable maternal, perinatal, family, societal, and global impacts of pre-eclampsia, and the other pregnancy complications. We aim to build sustainable, equitable partnerships across the individuals and institutions.

PRECISE AND PRECISE DYAD

PRECISE will establish a cohort of 10,000 pregnant women across East, West and Southern Africa. With their consent we will collect biological samples and information about their lives, contexts and stories. This information will provide a unique and holistic data set that will be used to investigate the potential for introduction of novel methods to assist with diagnosis and management of placental disorders. A major legacy will be the highly-phenotyped representative PRECISE cohort of West, East and Southern African women and the creation of an incomparable African-based biorepository for future hypothesis-generated and -generating research. The core activity will be to collect environmental, social, geographical, demographic, clinical, and biological data related to three populations of women.

Building upon the detailed pregnancy information and samples established by PRECISE, PRECISE-DYAD will follow up 4800 women who participated in PRECISE in The Gambia and Kenya, and their children. We aim to determine if, and how, pregnancies complicated by pregnancy hypertension, fetal growth restriction, stillbirth, and/or preterm birth, identify those more likely to avoid complications in subsequent pregnancies, mental ill-health, or early onset of cardiometabolic diseases. We will also assess the impact of these pregnancy disorders on the children’s physical, mental, and neurodevelopmental health until three years of age. We will explore the consequences for maternal health trajectories of caring for children with moderate-to-severe neurodevelopmental delay resulting from complicated pregnancies, and will interrogate interactions with social determinants of health, including environmental exposures on maternal and infant health.

The Arrowsmith UBC-University of Rwanda Partnership

The University of British Columbia’s Faculty of Medicine entered into a partnership with the University of Rwanda aimed in 2018. This collaboration will facilitate curriculum development, clinical pedagogy, service delivery, and strengthened research capacity to assist in the development of in-country specialist clinicians and educators who can continue to help develop capacity throughout Rwanda and regionally.

ENGAGE

The ENGAGE (ENhancinG vAGinal dElivery in Greece) Trial is taking place in twenty-two maternity units in Greece involving 20.000 to 25.000 births. Study interventions include implementation of HSOG (Hellenic Society of Obstetrics and Gynecology) guidelines on labor management and targeted training on cardiotocography, obstetric emergencies and the Robson criteria. The total duration of the study is estimated to be 22 months. The primary outcome assessed will be the change in caesarean section rate and the secondary outcomes will be maternal and neonatal morbidity and mortality. The study is expected to yield new information about interventions to reduce unnecessary cesarean sections in Greece. These results may inform efforts to improve the quality of maternal and neonatal care, particularly to reduce the high cesarean section rates in Greece, leading the way how they can be reduced worldwide.
