Overview

The Improving Diabetes Outcomes (IDO) Lab, directed by Dr. Faisal Malik, is dedicated to improving health care quality and the health of youth and young adults living with diabetes.

Our team is committed to developing and investigating the effectiveness of innovative, person-centered interventions that make a difference in the lives of young people living with diabetes. Our work to date has included the development and implementation of a novel clinic-based program to support an emerging adult’s transition to independence with diabetes management (Achieving Health in Emerging Adults with Diabetes Program), exploring the use of Instagram to provide diabetes education and peer support to adolescents, and examining the effectiveness of using financial incentives to support insulin administration and glucose monitoring in teens with type 1 diabetes.

In addition, our lab engages in studies examining the impact of social determinants of health on diabetes management, as well as clinical trials designed to make new pharmaceutical agents and diabetes technology available to children and young adults living with diabetes. We also participate in diabetes quality improvement collaboratives to support our mission to improve health care quality.

Achieving Health in Emerging Adults with Diabetes (AHEAD) Program
The majority of emerging adults (EAs) with type 1 diabetes (T1D) do not meet glycemic targets and experience suboptimal psychosocial outcomes. Few evidence-based interventions exist for this group, and fewer still target health care transition (HCT) readiness to support successful transition to independence with chronic disease management. To assist EAs in effectively managing the stressors and challenges of T1D in the context of their everyday lives, we need interventions that are innovative, developmentally targeted, and feasible to implement in the clinical setting.

Grounded in the self-determination theory of motivation and the Six Core Elements of HCT, the Achieving Health in Emerging Adults with Diabetes (AHEAD) Program is designed to meet the diabetes self-management, psychosocial, and HCT needs of EAs. The AHEAD Program works to meet EA needs by (1) providing tailored education based on self-identified gaps in diabetes knowledge, insulin management, and nutrition skills; (2) identifying and addressing psychological comorbidities through integrated mental health screening and care provided by an embedded AHEAD Program psychologist; (3) meeting HCT needs by delivering health care navigation education by an embedded AHEAD Program social worker; and (4) structuring visits to support a gradual transfer of responsibilities from the caregiver to the EA. In a large single-arm study conducted with real-world providers, the AHEAD Program improved hemoglobin A1c (A1C) and reduced diabetes distress in EAs with T1D.

Our grant funded by Breakthrough T1D (PI: Faisal Malik) builds on our prior experience and promising preliminary AHEAD Program results to carry out a comprehensive hybrid type 1 effectiveness-implementation study to determine: (1) the AHEAD Program’s effectiveness in improving glycemia and psychosocial outcomes in a pragmatic randomized controlled trial; (2) barriers and facilitators to AHEAD Program implementation and program fidelity in different clinical environments, from the perspectives of EAs and the diabetes care team; and (3) whether the AHEAD Program is cost-effective. If the AHEAD Program improves EA diabetes outcomes and is feasible to implement in different contexts, this study will result in a promising, scalable intervention that enhances the long-term health trajectory of this vulnerable population and offer an opportunity to expand the standards of care for EAs with T1D and their families.

InvesT1D: Promoting Adolescent Investment in Diabetes Care
Instilling healthy type 1 diabetes (T1D) management practices in adolescence can set a pattern into adulthood, improving long-term health while reducing future burden on patient and family quality of life, adolescent psychosocial development, and the health care system. Health psychology and economics research suggest that financial incentives can increase adolescent adherence to T1D self-management regimens.

Through a partnership with colleagues at Harvard University, we developed InvesT1D, a flexible, patient centered incentives intervention based on rigorous developmental research that included qualitative, ethical, and stated preference analyses. A pilot randomized controlled trial (RCT) demonstrated that InvesT1D was effective at improving adherence to self-management goals and time-in-range among adolescents using continuous glucose monitoring systems.

In this funded trial (PI: Davene Wright), we are working to determine the most efficacious financial incentive structure and to evaluate InvesT1D’s impact on time-in-range, HbA1c, adverse events, long-term health, diabetes distress, family conflict, health care utilization, and adolescent and caregiver quality of life. Our interdisciplinary team’s innovative approach includes an RCT, trial- and model- based economic evaluations (from the payer, health care, and societal perspectives), a policy analysis, the development of a decision support dashboard, and qualitative interviews with patient, health care payer, and health care delivery system stakeholders. Methodologic contributions of the InvesT1D study can make a significant contribution to the scientific approach for developing incentive interventions for a range of behavioral health-related conditions.

SEARCH Food Security Study
Youth and young adults with type 1 diabetes (T1D) or type 2 diabetes (T2D) experiencing higher rates of food insecurity than the general population. Qualitative and quantitative research increasingly suggests that food insecurity varies greatly day-to-day, across seasons, and in length from intermittent to persistent. Many of food insecurity’s sequelae, as well as behaviors impacting diabetes self-management (e.g., mental health, nutrition, physical activity) are also highly dynamic across short periods of time.

While conventional epidemiologic cohorts excel at capturing long-term experiences, they fall short in identifying day-to-day variation in food insecurity and its sequelae, and in characterizing the context of an individual’s experience of being food insecure. Thus, in collaboration with colleagues at the University of South Carolina, University of Colorado, and Seattle Children’s, we are involved in the continuation of the SEARCH Food Security Cohort Study (mPIs: Angela Liese and Jay Mendoza) to identify the temporal causal cascades and actionable and acceptable interventions for eliminating food insecurity.

The second phase of the SEARCH Food Security Cohort Study is a longitudinal mixed methods study that works to: (1) evaluate differences in real-time glycemic control between participants with varying levels of food insecurity, assessed by average daily time in range (TIR) via CGM over two 14-day time periods spaced 9 months apart; (2) evaluate the within-participant impact of food insecurity on TIR and intermediate paths using EMA methods, including dynamic structural equation models; (3) use longitudinal, qualitative methods to concurrently and deeply characterize the context of a subgroup of participants’ food insecurity experience and coping process through in-depth, one-on-one, repeated semi-structured interviews over 9 months. The study’s findings will contribute to future interventions and policies that are designed to interrupt the cycle of food insecurity and poor glycemic control and are effective for and acceptable to young people with diabetes.
